How to Apply UDID Card Online
Any disabled person who wants to apply online for this card can easily apply with the help of the process given below.
- First, you need to go to the official website of Swavlamban Card: http://www.swavlambancard.gov.in/
- After this, the website’s home page will open.
- On the home page, you will find the option “Apply for Disability Certificate & UDID Card”. You need to click on this option.
- Upon clicking, the registration form will open in front of you.
- Then, you need to carefully fill in all the information asked in the form.
- After this, you need to upload all your documents.
- Finally, you need to print out your form and send it to the CMO office/Medical Authority.
How to Apply Offline for UDID Card
